I am aware that baseball lessons are extreme workouts that are physically, mentally, and emotionally exhausting, and as a result, it is a potentially hazardous activity which may result in personal injury. I hereby assume all risks associated with of my or my child’s participation in any and all lessons and/ or workouts conducted by Complete Game LLC, its owners, members, employees, subcontractors, or consultants and performed at the above training facility, or at any other location requested by me (or parent as the case may be) for said lesson(s), including but not limited to any throwing related injury, swinging related injury, injury suffered from being hit by any balls, any fall downs or collisions, or contact with other participants, all risks being known and appreciated by me on my behalf on behalf of my child. I certify that I am (or my child is) medically able and sufficiently trained to participate in these lessons and workouts. Having read this release and knowing these facts and in consideration of your accepting my (or my child’s) as a student, I, for myself, my child, and anyone entitled to act on my behalf or his/her behalf, hereby waive, discharge and release Complete Game LLC and their respective agents, representatives, members , successors, subcontractors, consultants and/or assigns or any other persons acting on their behalf and agree to hold them harmless from and against any and all claims, complaints, counterclaims, demands, actions, causes of action, debts, liabilities, damages, costs, fees, expenses, rights, duties, obligations, liens, petitions, suits, losses, controversies, executions, offsets and sums, of any kind or nature, whether direct or indirect, whatever type or nature of any kind, that have or may arise out of my or my child’s participation in any training, lessons or workouts conducted by Complete Game LLC, or their respective agents, representatives, members, successors, subcontractors, consultants and/or assigns or any other persons acting on their behalf, including any personal injuries I or my child may suffer, even though the liability may arise out of negligence or carelessness on the part of the persons or parties referred to in this waiver.
